Navigating community supports during a difficult time shouldn’t feel overwhelming. SFEN’s FORT CARES model functions as Fort Erie’s central Community Navigation and Connection Hub—working much like an Air Traffic Control tower for local social services. Our mission is to provide essential, timely, and accurate guidance, ensuring every individual is directed to the exact support and resources they need.

How FORT CARES Works
When you are facing complex challenges, navigating multiple agencies can lead to stress, roadblocks, and confusion. FORT CARES acts as your central controller to ensure a smooth, safe, and organized journey toward stability.
1. Initial Assessment & Flight Plan
Whether you walk into the SFEN FORT CARES Hub or are referred by a partner organization, our friendly community navigators start with a supportive conversation. Together, we evaluate your immediate situation—including challenges related to housing, food security, employment, finances, tax assistance, or administrative paperwork. From there, we map out a practical “flight plan” to help you reach your desired destination, whether that is secure housing, a stable budget, or a completed application.
2. Preventing Collisions & System Hazards
Navigating bureaucracy alone can feel exhausting. FORT CARES actively works to remove systemic barriers, streamline processes, and prevent you from getting lost in the system. We work directly alongside you to clear the path forward.
3. Real-Time Advisories & Solutions
Our dedicated team provides clear, real-time guidance tailored to your specific situation. We focus on practical, actionable solutions to resolve immediate concerns without delay.
4. Inter-Agency Collaboration & Warm Handoffs
The true power of FORT CARES lies in our strong network. We do not replace specialized community services; instead, we maintain direct connections with the agencies funded and equipped to address specific, complex needs. We identify the right resource, coordinate directly with service providers, and facilitate a seamless, warm handoff so you never have to start over from scratch.
